Large extra-axial cystic lesion occupying the left anterior and middle cranial fossa causing mass effect in the form of midline shift and compression of anterior horns of both lateral ventricles. This is hypointense on T1 with hyperintense areas, hyperintense on T2 with incomplete suppression of signal on FLAIR and restriction on DWI images.

Case Discussion

A large extra-axial cystic lesion that does not follow CSF signal on all sequences and shows diffusion restriction is in keeping with an epidermoid cyst.
