Presentation

Painless swelling of the left mandibular region.

Patient Data

Age: 55 years
Gender: Male

Cone-beam CT

Expansile left mandibular lytic lesion centered on the ramus and mandibular angle with cortical thinning and destruction.

mri

On the MRI, this lytic lesion displays an isosignal to muscles on T1, high signal on T2 and FLAIR with restricted diffusion on DWI/ADC. The postcontrast sequences demonstrate a thin peripheral rim enhancement. No soft tissue component. Normal appearance of the adjacent muscles (medial pterygoid and masster).

Small cervical cord hydromyelia is noted (incidental finding).

Cone-beam CT and MRI features of a left mandibular lytic lesion with no soft tissue component and restricted diffusion on the MRI exam, pathologically proven as epidermoid cyst.
