Right scrotal pain since one week.

Patient Data

Age: 35

Ultrasound scrotum

Mildly enlarged righjt epididymis with heterogenous hyperechoic echotexture. The right testis and epididymis showed increased vascularity surrounded by multiloculated scrotal hydrocele.

Normal size and texture of the left testis and epididymis with minimal hydrocele.

Bilateral scrotal sacroliths are noted.

Case Discussion

Epididymo-orchitis is common cause of scrotal pain.  It can occur at any age. It should be differentiated from testicular torsion and follow up may be required to assess possible complications like abscess formation or testicular infarction. Focal testicular orchitis can appear indistinguishable from the possibility of malignancy on US basis. Treatment usually conservative with antibiotic course.
