Patient referred from the urological emergency complaining of pain in the right testicle for 5 days.

Thickened right epididymis, showing increased echogenicity and vascularity, associated with multiple cystic images of anechoic content in the tail region. Right testicle of enlarged dimensions heterogeneous echogenicity, and diffuse increase in its vascularization.
Case Discussion
Clinically, the patient had pain on examination of the testicular pouch with edema and local flushing. Refers sexual activity without using a condom.
