Severe scrotal pain with swelling
Patient Data
The examined testicle appears edematous, swollen with linear hypoechoic streaks of edema inside its parenchyma.
On Doppler interrogation; the testicle shows increased flow signals compared to contralateral side.
The epididymis appears swollen & enlarged in size with heterogenous hypoechoic texture & increased vascularity on color Doppler.
Thickened edematous overlying scrotal skin and subcutaneous tissue.
Prominent ipsilateral inguinal lymph nodes with thickened cortices likely reactive.
Case Discussion
Epididymo-orchitis is a common cause of scrotal pain. It can occur at any age. It should be differentiated from testicular torsion and follow-up may be required to assess possible, however rare, complications like abscess formation or testicular infarction.
Treatment is usually conservative with an antibiotic course.
