Presentation
Low-grade fever, restlessness, parents noticed left hemiscrotal swelling. On physical exam, the left testis appears swollen and hyperemic.

Some images are non-diagnostic and others are obtained at atypical angles due to the restlessness of the very young patient.
Both testes are homogeneous, the left shows slightly increased vascularity. The left epididymis is engorged and hypervascular. Significant hydrocele showing fine internal septations.
Case Discussion
Findings are in line with unilateral orchitis and epididymitis. Antibiotic treatment resulted in rapid recovery.
