Epidural lipomatosis in the lumbosacral transition. T1 high signal, T1FS shows fat suppression. Degenerative disc disease.

The arrow indicates the compression of the spinal canal. It resembles a "Y" shaped configuration.
Case Discussion
Spinal epidural lipomatosis can develop spontaneously or due to obesity as a result of disease and can cause compression of the spinal cord.
In this case, the patient's likely cause is obesity which resulted in the accumulation of epidural fat with consequent compression of the spinal canal.
The usual treatment is immediate laminectomy with the removal of epidural fat.
