Dotted line outlines the swollen epiglottis in the shape of a thumb, which gives the name of the thumb sign of epiglottitis. 

Case Discussion

Lateral x-ray of the neck of the a child presenting with clinical epiglottitis demonstrates swelling of the epiglottis which projects as a rounded soft tissue structure into the hypopharynx. It represents the so-called thumb sign.
