There is an esophageal outpouching just above the diaphragm, clearly separate from the gastric fundus - which is below the diaphragm.  On the earlier images, it was seen to fill fairly gradually with contrast, indicating the presence of retained food contents.

Case Discussion

This case demonstrates a typical epiphrenic diverticulum.  It is a pulsion diverticulum - or rather pseudodiverticulum as it lacks a muscularis layer.  Although there is no known or demonstrable motility disorder in this case, this is a frequent association of epiphrenic diverticula.
