Epiploic appendagitis

Presentation

Left iliac fossa pain and tenderness since for 4 days.

Patient Data

Age: 25 years
Gender: Male
ultrasound

In the left iliac fossa, there is a focal rounded hyperechoic lesion measured 1.4 x 0.9 cm, non-compressible, with no vascularity inside, surrounded by a hypoechoic halo and a central dot of hypoechoic indicative of a central thrombosed vein within the inflamed fatty lesion. It is located lateral to the distal descending colon, with a slightly thickened wall surrounding echogenic and thickened fat.

Case Discussion

According to its clinical symptoms, location in left iliac fossa, size, and sonographic imaging, mostly epiploic appendagitis.

DDx; omental infarction, usually larger, more located medial to bowel and in right iliac fossa.

Epiploic appendagitis is a self-limiting disease, it usually responds well to NSAIDs.
