Presentation

Left paraumbilical pain with tenderness

Patient Data

Age: 30 years
Gender: Male

There is an ill-defined non-compressible, hyperechoic area measuring (30 x 20 mm) adjacent to the wall of the proximal sigmoid colon and lateral to the left rectus abdominis muscle.

 On CT, it appears as an ovoid structure of fat-density, adjacent to the proximal sigmoid colon with a thin peripheral hyperdense rim "hyperattenuating ring sign" and surrounding inflammatory fat stranding. A central hyperdense dot well-visualised on zoomed images which may represent a thrombosed vascular pedicle.
