4 months of progressive gait disturbance, apathy and dementia.

Diffusely bilateral symmetrical widened perivascular spaces (Virchow-Robin spaces) in the basal ganglia, especially the corpus striatum, and also in the white matter bifrontally. They exhibit CSF-signal intensity, being hyperintense on T2-weighted images and hypointense on FLAIR images, without evidence of restricted diffusion.
