Bilateral symmetrical diffuse widened perivascular spaces in basal ganglia (especially corpus striatum) and thalami, forming cribriform cavities. They display hyperintense signal in T2-weighted images and exhibit CSF signal on FLAIR sequences, but not restricted diffusion on DWI/ADC.
Also noted are a left frontal lacune, surrounded by gliosis, and extensive microvascular-related changes.
Case Discussion
Profuse symmetrical widening of perivascular spaces throughout the basal ganglia in the elderly is described as status cribrosum, also known as état criblé.
