Large region of heterogeneous lucency in the lateral left iliac blade. This appears too lateral to represent overlying bowel gas, and there is peripheral cortical irregularity/destruction, suspicious for a destructive bone lesion.
Case Discussion
An 'edge of the film' diagnosis, highlighting the importance of tracing the cortices of all bones to check for subtle destruction. If more medial, this could have been mistaken for overlying bowel gas.
Histopathology confirmed Ewing's sarcoma.
