Lungs have been explanted from the donor and are being mechanically perfused and ventilated in preparation for transplantation into the recipient. Air-space opacity is seen bilaterally. 

Case Discussion

Ex vivo lung perfusion is a technique used to prepare lungs for transplantation. It is used in cases where the lungs are not appropriate for immediate transplantation due to poor oxygen transfer, pulmonary infiltrates such as edema, poor inflation and deflation or after cardiac death. The technique allows the lung function to be improved and objectively assessed prior to transplantation into the recipient, and means that a greater number of donor lungs can now be used than was the case prior to the advent of this technology.
