Presentation

Anterior head trauma on car accident

Patient Data

Age: 45 years
Gender: Female

Bilateral displaced fractures of nasal bones, notably the right side.

Fissure fracture is seen through the anterior maxilla.

Fracture anterior-inferior corner of C2 vertebral body, reflecting extension tear drop fracture. No associated hangman fracture.

Case Discussion

Extension teardrop fracture is an avulsion fracture of the anterior inferior corner of the vertebral body due to forced neck extension. It is stable in flexion and unstable in extension as the anterior longitudinal ligament is disrupted. It is treated conservatively with immobilization (C spine collar).
