Extensive cavernous transformation of portal vein

Presentation

Patient presented with a history of previously thrombosed portal vein.

Patient Data

Age: 40 years
Gender: Male

The main portal vein, superior mesenteric, and splenic veins are not demonstrated. There are many dilated and serpiginous vessels not only around the native portal vein but also around the native superior mesenteric and splenic veins - cavernous transformation. Marked splenomegaly.

Prominent esophageal, paraesophageal, gastric, perisplenic, omental, and mesenteric portosystemic collaterals.

No cirrhotic changes of the liver.

Case Discussion

Formation of venous channels around previously thrombosed portal vein consistent with cavernous transformation. Also, superior mesenteric and splenic veins are not demonstrated. In this case, the absence of normal portal circulation makes it impossible for successive surgical shunting.

This is a typical case of non-cirrhotic portal hypertension.
