Extensor hallucis longus tendon re-tear

Presentation

Two months before the presentation, a patient had cut wound to the dorsum of the foot. The extensor hallucis longus tendon was repaired. Due to poor great toe extension after two months of the tendon repair, ultrasound was requested to check the tendon integrity.

Patient Data

Age: 35 years
Gender: Male
ultrasound

There is a discontinuity of the repaired extensor hallucis longus tendon. The defect between the two edges of the tendon is about 45 mm. The proximal end of the tendon is at the talonavicular joint region and the distal end the first metatarsal base level.

There loss of definition of the dorsalis pedis arterial wall at the scar region with the absent flow in the proximal segment. The distal arterial segment shows a low-velocity slow flow.

Surgical sutures are present in the scar region.

The case shows the re-tear of the extensor hallucis longus tendon and thrombosis of the dorsalis pedis arterial segment.
