Extensor hood injury at the metacarpophalangeal joint

Case contributed by Dr Roberto Schubert

Pain and snapping sensation over the 3rd MCP joint after a punch in a professional boxing athlete.

Patient Data

Age: 25 years
Gender: Male

Radial displacement of the 3rd finger extensor tendon with thickening and high T2 signal edema surrounding the extensor hood.  The ulna sagittal band is discontinuous at the joint level and findings likely in keeping with rupture of the ulna sagittal band of the extensor hood at the level of the 3rd metacarpophalangeal joint.

Case Discussion

Ruptures of the sagittal bands of the extensor hood can occur from single or repetitive traumata or forceful hyperflexion of the MCP joint. Associated lesions of the MCP joints are frequently encountered (Boxer knuckle).
