Extensor tenosynovitis of wrist

Diagnosis almost certain

Presentation

Elongated swelling on the dorsal aspect of the right wrist for the last 3 months. No trauma.

Patient Data

Age: 25 years
Gender: Female
x-ray

There is lobulated soft tissue swelling on the dorsal aspect of the wrist. There is no bone erosion/ periosteal reaction. There is no fracture/ dislocation/ bone lesion.

ultrasound

The clinically visible swelling is due to 4th extensor compartment tendon sheath effusion and synovial hypertrophy which is associated with hypervascularity. The tendons show normal fibrillar echopattern without tendon tear. The rest of the extensor compartments were normal. There is no joint effusion/ joint synovial hypertrophy. There is no bone erosion.

Case Discussion

The case shows tenosynovitis of the 4th extensor compartment of the wrist. Tenosynovitis is a non-specific finding which may occur due to trauma, mechanical irritation, rheumatoid arthritis, gout, CPPD, infection, diabetes, etc.
