Extra-abdominal desmoid tumor-shoulder

Presentation

Large shoulder mass with pain and restriction of movement.

Patient Data

Age: 40 years
Gender: Female

There is a large mass lateral to the humerus, this mass appears inter-muscular (interposed between the deltoid and triceps brachii muscles) and invades the subcutaneous fat with irregular infiltrative margins, it appears isointense on T1 and hyperintense on T2 with marked enhancement after IV contrast administration.  
Hypointense bands within the mass, which represent collagenized bands; these bands lack prominent enhancement on the postcontrast images. 
This mass measures about 6.8 x 4.5 x 9.3 cm (AP.TRANS.CC).

Features are in keeping with an extra-abdominal desmoid tumor. 

No evidence of bony invasion. Normal alignment of the left arm. No bony fractures or bony masses. 

Case Discussion

Although they are benign, extra-abdominal desmoid tumors are aggressive and locally invasive lesions. They are usually seen in young adults, with a high prevalence of recurrence and need aggressive treatment by wide surgical resection and even chemotherapy and radiotherapy.
