Low back pain with referral to the left leg. Reduced straight leg raise. Dampened ankle jerk, L5/S1 weakness. Disc prolapse?

Normal segmentation.
Large L5/S1 left paracentral disc extrusion, filling the left lateral recess and indenting the thecal sac. This is impinging on the descending left S1 nerve root.
No impingement of the exiting L5 root.
The remaining levels are normal in appearance.
L1 conus.

A large lumbar disc extrusion with the toothpaste sign illustrated.
This is a typical example at one of the commonest levels along with L4/L5.
