Eyelid abscess

Presentation

Abscess of the right upper eyelid with marked adjacent soft tissue swelling. Involvement of the orbital tissues?

Patient Data

Age: 40 years
Gender: Female

An about 15 x 12 x 7 mm fluid signal circumscribed lesion is visible in the subcutis of the right upper eyelid, in line with an abscess. The adjacent marked diffuse soft tissue edema with increased contrast enhancement is best appreciated on the STIR sequences.

Discrete post-septal high STIR signal adjacent to the lateral and superior rectus muscles with increased contrast enhancement, but no unequivocal sign of frank post-septal involvement. 

Incidental bilateral maxillary sinus retention cysts. 

Case Discussion

Topical and systemic antibiotic therapy resulted in rapid recovery. Post-septal orbital involvement was not confirmed by clinical course and no further imaging was done.
