Diagnosis almost certain

Presentation

Young female presented with a severe headache and blurred vision.

Patient Data

Age: 20 years
Gender: Female

MR demonstrates bilateral T1 hyperintense signal within the bilateral basal ganglia, posterior aspect of the thalamus and cerebellar dentate nuclei. There is also mild hyperintense signal at same levels on T2WI and FLAIR. No diffusion restriction was noted.

Unenhanced CT reveals dense calcifications within the bilateral basal ganglia, posterior aspect of the thalamus and cerebellar dentate nuclei.

Findings most suggestive of Fahr disease.

Case Discussion

In this case, the absence of metabolic and infective diseases confirm the diagnosis of Fahr disease.

The CT imaging the most sensitive modality to confirm and assess the severity of calcifications.
