Failed early pregnancy

Case contributed by Huy Sruy
Diagnosis certain

Presentation

Young woman presents with suprapubic pain.

Patient Data

Age: 25 years
Gender: Female
ultrasound

CRL (Crown rump length) is 20 mm on these transabdominal views and no heartbeat is seen.

Case Discussion

To minimize the risk of false positive diagnosis of miscarriage:

  • a mean sac diameter (MSD) cut off >25 mm

and/or

  • a crown rump length (CRL) cut off >7 mm

without a detectable heart rate is compatible with early pregnancy failure.

Please note that these values are for endovaginal (transvaginal) imaging. If transabdominal imaging only, then the CRL cut off is >15 mm.
