Fallen fragment sign

Case contributed by Ian Bickle
Diagnosis almost certain

Presentation

Fall. Pain. Fracture?

Patient Data

Age: 8 years
Gender: Male
x-ray

Displaced fracture through a mildly expansile lucent intramedullary lesion with a narrow zone of transition in the proximal humeral diaphysis.

A small fallen fragment lies in the dependent position of the lesion.

Case Discussion

A wide differential exists for lucent bone lesions, one of which is a simple (or unicameral) bone cyst.

This can result in a pathological fracture with the classical appearance being that of a fallen fragment when a small bone fragment 'sinks' into the dependent position of the cyst.
