Femoral head avascular necrosis - photopenia (bone scan)

Presentation

Severe lower back pain, worsening with degenerative facets on MRI ? facet joints amenable to injection. L3-5 laminectomy.

Patient Data

Age: 60 years
Gender: Male

Procedure: A 3 phase bone scan and SPECT/CT of the lumbar spine and pelvis were performed following IV injection of 800 MBq of Tc-99m MDP.

Nuclear medicine

No blood flow or blood pool abnormality in the lumbar spine.

There is mild degenerative osteoblastic activity at the L5/S1 endplates.

Mildly increased osteoblastic activity at the L4/5 right facet joint.

Photopenia of the superior left femoral head correlates to geographic sclerosis on CT compatible with avascular necrosis.

L3-5 laminectomy with minor retrolisthesis at L5/S1.

Incidental note of mild, uncomplicated diverticular disease.

Case Discussion

Mild degenerative osteoblastic activity at L5/S1. Mild osteoblastic activity associated with L4/5 right facet arthropathy. Left femoral head avascular necrosis.
