Presentation
Conductive hearing loss.
Patient Data
Hypodensities in the lateral wall of the bony labyrinth in the region of fissula ante fenestram and cochlear promontory. No pericochlear involvement. Sclerosis at both the oval and round windows bilaterally. On the left, hypodensity involves the lateral wall of the tympanic segment of the facial canal.
Middle ears clefts are clear bilaterally. Ossicular chains are congruent. No dehiscent or high riding jugular bulbs. Inner ear structures appear normally formed. No IAC widening or obvious IAM mass.
Visualized intracranial structures, orbits, paranasal sinuses and the right neck demonstrate no gross abnormality.
Case Discussion
This patient had a known history of otosclerosis with conductive hearing loss. The CT demonstrates a fenestral distribution.
