Fibroid uterus and ovarian fibroma

Post menopausal bleeding

Patient Data

Age: 60 years
Gender: Female

The uterus is anteverted and enlarged by fibroids measuring 94 x 63 x 68 mm (volume 211 mL). The largest fibroid is submucosal at the fundus with significant impaction onto the endometrial cavity, measuring 37 x 33 x 36 mm. 

The endometrium is thin. Its outline is distorted by fibroids. A small amount of fluid in the cavity also outlines the impact of the fundal submucosal fibroid. 

The right ovary measures 27 x 14 x 12 mm (volume 2.4 mL) and contains no follicles. The ovary is mobile and non tender. 

The left ovary measures 27 x 17 x 22 mm and contains no follicles but contains a round avascular solid mass with acoustic shadowing. This measures 17 x 22 mm. The ovary is mobile and non-tender. 

No free fluid in the pouch of Douglas. 

Case Discussion

Multifibroid uterus with one of the fibroids significantly impacting on the cavity. This could at least in part be contributing to post menopausal bleeding. There is no obvious endometrial pathology.

2 cm left ovarian solid lesion has the ultrasound appearance of a fibroma.
