Cortical desmoid

Presentation

Hip pain. Incidental finding on plain radiographs.

Patient Data

Age: 7 years
Gender: Female
x-ray

Poorly defined lytic lesion in the posterior femoral metaphysis.

T1 hypointense and T2 hyperintense, lobulated, cortically-based mass (interestingly despite being histologically fibrous they still have increased T2 signal).  It is much more circumscribed on the MRI.

Case Discussion

These are considered to be benign lesions that are self-limiting. They can also be seen following repetitive trauma and are then known as cortical desmoids (due to mechanical pull by the medial head of the gastrocnemius).
