Presentation

The patient presented with a twisted ankle after coming off the trampoline and has pain and swelling in the foot and ankle, unable to bear weight.

Patient Data

Age: 15 years
Gender: Female

There is a minimally displaced intra-articular fracture through the base of the right second metatarsal.

Case Discussion

Bony fragment visible (“fleck sign”) in the space between the 1st and 2nd metatarsal, indicates avulsion of the Lisfranc ligament from the base of the second metatarsal.

Disruption of a line drawn from the medial base of the 2nd metatarsal to the medial side of the middle cuneiform (on AP view).
