Flexor carpi ulnaris - calcific tendinitis

Presentation

Acute onset, the atraumatic origin of severe right wrist pain involving palmar-ulnar side - 2 days.

Patient Data

Age: 60 years
Gender: Male

Edema involving the palmar ulnar side near the right wrist.

Tiny echogenic foci in the distal end of the flexor carpi ulnaris tendon and peritendinous soft tissue. Associated local soft-tissue edema and a small amount of fluid. Intact tendon. No cortical erosion of the adjacent pisiform.

Asymptomatic side flexor carpi ulnaris tendon with a normal echopattern without peritendinous soft tissue thickening/fluid.

Case Discussion

Clinical presentation and ultrasound findings favor calcific tendinitis of the flexor carpi ulnaris.

Palmaris longus and flexor carpi ulnaris are only two tendons at the wrist without a synovial sheath. Hence, the term 'tenosynovitis' should not be reported when there is fluid around these tendons.
