Flexor carpi ulnaris partial tear and hypothenar muscle group strain

Presentation

Trauma.

Patient Data

Age: 35 years
Gender: Female

Abnormal intrasubstance increased fluid signal along the course of flexor carpi ulnaris musculotendinous junction and tendon next to insertion on the pisiform bone due to partial tear.

There is also abnormal intrasubstance increased fluid signal along the course of the hypothenar muscle group (flexor digiti mini brevis and abductor digiti minimi) due to hypothenar muscle group strain.

Bone marrow edema/bruise of pisiform next to the insertion of flexor carpi ulnaris tendon and origin of the hypothenar muscle group.

Small synovial cyst measuring about 3*5 mm originates from the triquetrum-pisiform joint space.

Case Discussion

On physical examination, the pain is noticeable over the pisiform bone, the distal portion of the flexor carpi ulnaris tendon, and the hypothenar group muscle during resisted wrist flexion and ulnar deviation. Regarding the MRI findings, flexor carpi ulnaris partial tear and hypothenar muscle group strain is the main diagnosis.
