Chronic pelvic pain.

Age: 50 years

The MR sequences demonstrate an enlarged uterus with a focal thickening of the endometrium-myometrium junctional zone at posterior fundal/corporeal region (thickness=44 mm), seen as an ill-defined area of intermediate signal on T1WI with foci of high signal (menstrual hemorrhage into the ectopic endometrial tissues), low signal on T2, containing areas of high signal, compressing the endometrial cavity which is displaced anteriorly. 

Case Discussion

MRI of features of focal adenomyosis. The main differential diagnosis is a uterine leiomyoma (peripheral displacement the vessels on color Doppler +/- calcification).
