Recurrent seizures.
Patient Data
The right superior temporal gyrus shows focal cortical thickening and subcortical gray/white matter blurring with altered bright signals at T2& FLAIR while low T1 signal. No diffusion restriction. Additionally, a connection from this region to the right ventricle appears as a high signal linear band (transmantle sign ).
Case Discussion
The clinical history of recurrent seizures, young age, with the main abnormality being cortical & subcortical high T2 signal having the classic appearance & location makes focal cortical dysplasia the most likely diagnosis.
According to Blumcke's classification of focal cortical dysplasia, this case matches type II with the presence of transmantle sign.
