Focal fatty sparing of the liver

Presentation

Right upper quadrant pain for investigation

Patient Data

Age: 75
Gender: Female
CT

Non-contrast scan as patient has had previous iodine allergy

Diffuse fatty liver (HU density 23) with well-defined "mass" in gall bladder fossa (HU density 50)

Ultrasound

US for confirmation

Typical increased echogenicity of liver consistant with steatosis and focal sparing (normal liver) at the site of CT abnormality. Normal blood flow on Doppler.

Case Discussion

The gall bladder fossa/porta hepatis is a typical location for focal fatty sparing in patients with steatosis. It can sometimes be confused with a mass lesion. It has features of normal liver on CT and US
