Focal fatty sparing of the liver

The liver is enlarged, measuring about 18 cm-length and shows severe fatty infiltration with sparing areas near the porta-hepatis.
Case Discussion
There are typical hypoechoic areas with geographic appearance near the porta hepatis, with absent mass effect and distortion of vessels that run through the region. The reporter may believe these areas to abnormal, however, regions of focal sparing may be seen around focal liver lesions.
