At the foramen magnum anteriorly, there is an extra-axial mass with low signal intensity on T1 weighted images and mildly hyperintense on T2/FLAIR weighted images. The mass reveals homogeneous enhancement on post-contrast injection with dural tail. There is significant mass effect and compression of the medullo-cervical region posteriorly. No hydrocephalus is seen.

Case Discussion

Findings are in line with an extra-axial dural-based mass, which is most likely a meningioma. The location within the foramen magnum resulted in early compression of the adjacent brainstem.
