Foraminal disc prolapse

Diagnosis almost certain

Presentation

Pain the left thigh. No regression in pain and now numbness.

Patient Data

Age: 65 years
Gender: Male
mri

Normal vertebral body heights. Normal bone marrow signal.

Small focal left foraminal disc prolapse at L2/L3, impinging on the L2 root in the origin of the foramen.

The remaining levels are normal in appearance.

L1 conus. Normal cauda equina.

 

Case Discussion

Left L2/L3 foraminal disc with L2 nerve root impingement.  The findings correlate with the symptoms of pain and numbness in the thigh.

Foraminal disc prolapses can easily be overlooked at they are small, but this is all it takes in the confined space of the exit foramen to cause nerve root impingement.

In this case, observe the asymmetry of the exit foramina at this level compared to the normal right side.

Ensure the foramen in a review area on reporting lumbar spine MRI.
