Mechanic with localized sudden pain and bleeding from forearm for past 3-4 days while working. Complains of mild residual local pain. US to rule out foreign body.

Echogenic focus in subcutaneous fat abutting a non-thrombosed superficial vein, showing twinkle artifact on Doppler interrogation. Size - 1 mm. No local collection or hypoechoic granulation around foreign body.
Case Discussion
Findings favor metal foreign body in forearm. Similarly to a renal calculus, a metal foreign body causes a twinkle artifact that helps to localize it.
