Presentation
History of penetrating injury by wood splinter to the dorsum of the left hand at 5th metacarpophalangeal joint about 4 weeks back. Complaining of local pain, swelling without local discharge.
Patient Data
There is a well-defined swelling on the dorsal aspect of hand near the 5th metacarpophalangeal joint. There is no sinus.
There is a linear echogenic focus ( 13 mm ) on the dorsal aspect of hand; aligned along 5th metacarpus long axis. It overlies the distal end of the 5th metacarpus. It is a foreign body. It is surrounded by elongated hypoechoic soft tissue thickening; measuring 23 x 7 x 6 mm. It is superficial to intact extensor tendons. It is a granuloma. There is local hypervascularity. There is no tendon tear/ joint effusion/ underlying cortical erosion.
Case Discussion
A young male presented with a history of wood splinter injury to the dorsum of the hand. Ultrasound shows foreign body granuloma at the site of clinically visible swelling. It was removed surgically.
