Fournier gangrene

Case contributed by Dr Rodney Strahan

Diabetic patient lanced a perineal abscess in a remote African village. Hours later presented in shock with a fever.

Patient Data

Age: 80
Gender: Female

Gas is tracking from the perineum along fascial planes up the left flank.

Dilated small and large bowel, probably due to an ileus caused by the nearby infection.

Case Discussion

Fournier gangrene typically occurs in male diabetics, rarely in females. It is an emergency to debride the affected tissue and treat the infection.

This patient had extensive tissue involved, due to the time to reach medical help, and did not survive.
