Complex cystic mass immediately lateral to left hemithyroid.

Cyst lateral to left hemithyroid, deep to sternocleidomastoid, displaces IJV anteriorly, CCA posteriorly.
Case Discussion
This cyst was aspirated. Cytology shows benign squamous cells, neutrophil polymoprhs, amorphous acellular debris. These findings support diagnosis of a branchial cleft cyst, with the position typical of a very rare fourth branchial cleft cyst.
