Presentation

Sudden left hemiparesis in a hypertensive patient.

Patient Data

Age: 70 years
Gender: Male
mri

Acute infarct involving the inferomedial aspect of the right hemi-pons seen as an area of low signal on T1, high signal on T2 and FLAIR with restricted diffusion. No hemorrhagic component was seen on the GE sequence.

Cerebral volume loss with small vessel ischemic changes are noted.

Case Discussion

MRI features most consistent with Inferior medial pontine syndrome, also known as Foville syndrome, and considered as one of the brainstem stroke syndromes. The infarction involves the medial inferior aspect of the pons secondary to occlusion of the paramedian branches of the basilar artery.
