Recurrent urinary tract infections.

There is an isolated dilatation of the right upper calyx on the renal cortical and excretory phases.

The coronal reformatted renal excretory phase (MIP) shows a well-defined horizontal filling defect in the superior infundibulum along with distension and delay in emptying of the upper calyx group.

VR Reconstruction images confirm the extrinsic vascular compression of the upper calyceal infundibulum by a branch of the right renal artery causing distension and dilatation of the upstream calyx.

Case Discussion

Fraley syndrome is a rare cause of obstruction of the collecting system. It results from an extrinsic vascular compression of a calyceal infundibulum by a branch of the renal artery. The upper right calyx is the most commonly involved.
