Diagnosis certain

Presentation

Right sided abdominal pain. Unknown cosmetic procedures performed in another country several years ago.

Patient Data

Age: 60 years
Gender: Female

CT of the abdomen and pelvis shows multiple nodular densities, some with peripheral hyperdensities, in the bilateral breasts and buttocks. Findings are consistent with free silicone injections/fillers in both breasts and buttocks.

Case Discussion

This case shows imaging findings of free silicone injections in the bilateral buttocks, also partially imaged in the bilateral breasts. Free silicone injections are not approved for cosmetic purposes by the FDA in the USA, but may be performed illegally or in other countries.

Silicone complications can be divided into focal (cellulitis, abscess, myositis, migration of the material, skin hyperpigmentation, fibrosis, subcutaneous nodules, granulomas, necrosis, ulceration, fistula) and systemic (hypersensitivity pneumonitis, silicone embolism syndrome, pulmonary edema, sepsis).
