There is flattening, lucencies and fragmentation of the head of the right second metatarsal in keeping with avascular necrosis.

Case Discussion

Acute or repetitive injury and vascular compromise is postulated as a possible causative factor, leading to subchondral collapse and osteonecrosis of Freiberg disease.
