Frontotemporal dementia: right temporal variant

Diagnosis almost certain

Presentation

12 month history of progressive memory impairment associated with disinhibition, change in social conduct with inappropriate behavior, perseveration, impulsive, lack of insight and unable to recognize and distinguish speaking tone of people. Very poor result in Cambridge Face Memory Test. NuCOG significant impairment in memory; relative preserved executive function.

Age: 56
Gender: Female

Global atrophy with frontotemporal predilection. No occipital atrophy. Significant more right temporal then left temporal atrophy. 

Case Discussion

56 years old female who presented with both personality/behavior change consistent with frontotemporal dementia of behavioral variant, but at the same time also has significant memory impairment evidenced by objective cognitive assessment. Classically behavioral variant of FTD does not have significant memory impairment and less so temporal lobe atrophy. Right temporal variant of FTD is consistent with the clinical finding and MRI appearance.
