Gallbladder adenocarcinoma

Presentation

Right upper quadrant pain.

Patient Data

Age: 70 years
Gender: Female
mri

Distended gallbladder containing two stones with sludge forming fluid-fluid level. Large lobulated soft tissue mass involving the neck and body of the gallbladder of low signal on T1, intermediate signal on T2 and FIESTA with restricted diffusion and heterogeneous enhancement on postcontrast sequences. No apparent extension to the adjacent segments of the liver (segments 4 and 5). The tumor is in contact with the duodenal wall with no fatty interface (probably invaded).

Periportal lymphadenopathy with central necrosis and restricted diffusion.
