Gallbladder carcinoma

Diagnosis almost certain

Presentation

Chronic right upper quadrant discomfort. Abnormal liver function tests, otherwise normal serum lab values. No past medial/surgical history.

Patient Data

Age: 50 years
Gender: Female

Complex loculated infiltrating liver mass measuring up to 12 cm, infiltrating segments V and VI. 

3 cm lamellated calculus within gallbladder.

Perihepatic fat stranding and several enlarged necrotic portocaval lymph nodes.

Portal vein is patent.

Infiltrative process extending from gallbladder into right lobe of the liver. It extends through the liver capsule anterolaterally with adjacent fat stranding, concerning for peritoneal involvement.

Case Discussion

This case presents an infiltrative process involving the gallbladder and liver, suspicious for gallbladder carcinoma extending to the liver. A hepatic neoplasm extending to the gallbladder is in the differential; however, significantly less likely.

Liver biopsy was performed and the histopathology report confirmed biliary adenocarcinoma.
